Alpha Omega's Life House provides hospital to home care for veterans who are medically fragile due to recent hospital stays.

Each veteran has a treatment team staff that includes Alpha Omega staff working with an assigned social worker and addictions specialist from the Veteran's Administration Medical Center. This treatment team reviews the veterans’ progress on a monthly basis for the duration of their stay in the Life House.

 The professional nursing staff is on call 24 hours a day.

 

The dedicated staff provides services such as:

• Medication adherence monitoring
• Pain management
• Assistance with activities of daily living
• Nursing assessments
• Education about their medications and disease process
